Declare 2.1 Recycled Content Questionnaire

When is this questionnaire required?

This questionnaire is required only when a manufacturer chooses to report the Recycled Content Range (Optional) field in Declare 2.1. If the product does not contain recycled material, or the manufacturer does not choose to report the optional field, this questionnaire is not required.

How to complete and submit this questionnaire:

Manufacturers that choose to report this field must complete one questionnaire for each distinct recycled material used in the product. A recycled material may be composed of multiple substances; a separate questionnaire is not required for each substance that makes up the same material.

Example - recycled newspaper would be a single material composed of several substances including cellulose pulp, lignin, calcium carbonate, kaolin, starch, pigments, oils, etc. One questionnaire would be filled out for the recycled newspaper material.

1. Complete one questionnaire for each recycled material.
2. Download the questionnaire responses emailed to you on completion of the questionnaire.
3. Upload the completed questionnaires in the Geofund platform with your label submission.

The information provided in the questionnaire is supporting documentation of the reported recycled content and will not be published on the Declare label or website.
