Declare Annual Third-Party Verified No-Change Confirmation Form

The Annual 3PV No-Change Confirmation Form documents the annual check-in for products with an active Declare Third-Party Verification (3PV) designation. The manufacturer and Verifier confirm whether any changes have been made that could affect information disclosed on the Declare label.

This confirmation is a brief annual check-in and does not replace the required full re-verification at the end of the three-year verification period. If changes are identified, they must be reported to Living Future and reviewed in accordance with Declare requirements.

The completed confirmation form is expected to be retained as part of the product’s third-party verification record and made available to Living Future upon request.
